High-Altitude Sickness (AMS) Guide for Overlanders: Prevention, Symptoms & Emergency Protocols
Overland expeditions through high-altitude corridors like Leh-Ladakh, Spiti Valley, and Sikkim expose drivers and passengers to extreme elevations ranging from 10,000 feet to over 19,000 feet at Umling La. As atmospheric pressure drops, available oxygen molecules per breath decrease significantly.
Without proper acclimatization and medical awareness, Acute Mountain Sickness (AMS) can turn an exciting mountain drive into a medical emergency. Here is what every overlander must know about high-altitude medical safety.
1. What Happens to the Body at High Altitude?
At sea level, effective oxygen concentration is 21%. At 11,500 feet (Leh), effective oxygen drops to roughly 14%. At 18,380 feet (Khardung La), effective oxygen plunges to around 10%.
The human body requires time to compensate by increasing breathing rate, heart rate, and red blood cell production. Ascending faster than your body can adapt triggers mountain sickness.
2. Recognizing the Three Stages of Altitude Sickness
Stage 1: Mild Acute Mountain Sickness (AMS)
Commonly occurs 6 to 24 hours after arriving above 9,000 feet.
- Symptoms: Throbbing headache, lightheadedness, loss of appetite, nausea, mild insomnia, fatigue.
- Action Required: Stop ascending. Rest at current altitude, drink fluids, take mild analgesics (Paracetamol/Ibuprofen).
Stage 2: High-Altitude Pulmonary Edema (HAPE)
Fluid accumulation in the lungs. A life-threatening emergency.
- Symptoms: Breathlessness at rest, persistent dry cough producing pinkish/frothy sputum, blue lips/fingernails (cyanosis), extreme weakness.
- Action Required: IMMEDIATE DESCENT to lower altitude (drop at least 1,500 to 3,000 feet). Administer continuous medical oxygen.
Stage 3: High-Altitude Cerebral Edema (HACE)
Fluid accumulation in the brain. A life-threatening emergency.
- Symptoms: Inability to walk in a straight line (ataxia), confusion, slurred speech, hallucinations, loss of consciousness.
- Action Required: IMMEDIATE DESCENT. Administer high-flow medical oxygen. Seek emergency military or civilian hospital care immediately.
3. Golden Rules of High-Altitude Acclimatization
- Ascend Slowly: Never gain more than 1,000 to 1,500 feet of sleeping altitude per day once above 9,000 feet.
- Mandatory Rest Days: Spend at least 48 full hours resting in Leh (11,500 ft) or Kaza (12,500 ft) before crossing higher passes like Khardung La or Kunzum Pass.
- Climb High, Sleep Low: Drive up to higher viewpoints during the daytime, but return to lower elevations to sleep.
- Hydrate Continuously: Drink 3 to 4 liters of water daily. Cold mountain air dries out lungs and speeds up dehydration.
- Avoid Alcohol & Sedatives: Alcohol depresses breathing rate and worsens nighttime hypoxia.
4. Medical Gear Every Expedition 4x4 Should Carry
When driving remote circuits like Hanle, Zanskar, or Chushul, your vehicle must carry specialized medical equipment:
- Digital Pulse Oximeter: Check SpO2 (blood oxygen saturation) levels twice daily. Normal sea-level reading is 95-100%. Readings between 80-90% at 11,500 ft are expected; readings dropping below 75% require immediate rest and oxygen support.
- Portable Medical Oxygen Cylinders: Carry 10L-15L medical oxygen cylinders equipped with regulator valves and non-rebreather masks.
- Prescription Prophylactics: Consult a physician regarding Acetazolamide (Diamox) prior to departure. Diamox forces kidneys to excrete bicarbonate, stimulating faster breathing to aid acclimatization.
